GUIDELINES FOR APPLICATION FOR D9 SCHOLARSHIP FUNDS

Applications must be received by October 6, 2024 (at Midnight). For questions, please email tna9@tnadistrict9.com or call the D9 office at 713-523-3619.

INSTRUCTIONS

Scholarship applications must be submitted online using the form below and include the following information:

  • Cover Page – A number will be assigned to all applications and the cover page removed so that all reviews will be blind.  At least three reviewers will read each application.
  • Applicant Information
  • Scholarship Agreement
  • Copy of current transcript from all classes, including current nursing program with cumulative GPA (photocopies are acceptable)
  • Narrative – Address the following items in 5-6 pages.
    • Career Goals using the SMART Goal Format
    • Plan to Achieve Career Goals
    • Professional and Organizational Activities
    • Financial Need
    • Contribution of this Scholarship to Achieving Career Goals
  • Appendices –  Must include the following:
    • Resume or Vitae
    • Two letters of recommendation from faculty teaching courses in the nursing curriculum and written within the current year.

Sabina Fritsch Community Health Scholarship

One TNA District 9 scholarship will be awarded to a student (undergraduate or graduate) with a background in public or community health and/or an interest in a career goal of public or community health and /or ambulatory care.

Sabina Fritsch was the executive director for the Visiting Nursing Association (VNA) and had 36 years of service with the organization.  When the VNA experienced financial difficulty, Ms. Fritsch was the individual that the provided leadership needed to survive the “lean times” during the 1930s and become an independent organization no longer under the authority of the city (Houston).  During her tenure with the VNA, she guided the organization from an emphasis on disease management services to incorporating health promotion/disease prevention, a new concept in medicine during that time.

When completing the application for this specific scholarship, please include any experience (i.e., nursing, patient education, volunteer work) in public health, community health, hospice, home health, and ambulatory care, as well as the discussion of career goals and plans in these areas.

Carol Coats Mental Health Scholarship

One TNA District 9 scholarship will be awarded to a student (undergraduate or graduate) with a background in mental health and/or an interest in a career goal of mental health.

Carol Coats was a mental health advocate, working over 20 years as a psych nurse. She also served as an Addictions Counselor and TPAPN peer support advocate for nurses recovering from alcohol and substance use. She was a mentor, guide and friend to all who met her, donating numerous hours to counsel and support others.

When completing the application for this specific scholarship, please include any experience (i.e., nursing, patient education, volunteer work) in mental health, as well as the discussion of career goals and plans in these areas.

NOTE:  NO Direct Funds will be distributed.
