Evergreen Private Care is a private pay (non-medicare, non-medicaid) Home Health organization. We are growing and are looking for a strong RN as a Clinical Care Coordinator. This position would be responsible for managing a client load: overseeing their care, performing new client assessments, re-assessments, ensuring the customized care plan is being followed and that the client is satisfied with the care being delivered. Customer satisfaction is crucial. Establishing a strong client/family relationship positively impacts outcomes and is very rewarding!
