Nomination Form for OUTSTANDING NURSE Award 
Honoring remarkable nurses and presenting scholarships and research grants to blessed nurses and nursing students in District 9

District 9 of the Texas Nurses Association is accepting nominations for our 32st Annual Nursing Celebration 2024 OUTSTANDING NURSE Award.

We invite you to nominate registered nurses who have made an outstanding contribution to the profession—staff nurses, educators, managers, practitioners, community nurses, and school nurses.

Any registered nurse currently licensed in the state of Texas and working as an RN in the District 9 area (includes Brazoria, Chambers, Fort Bend, Galveston, Harris, Liberty, Montgomery, San Jacinto, Trinity, Walker and Waller counties) is eligible. Anyone may nominate a registered nurse; however, self-nominations are not eligible. Previous recipients are not eligible to receive the award again but may be nominated for the Lifetime Achievement Award at https://tnadistrict9.com/dorothy-otto-lifetime-achievement-award/ 

Nominee must send an e-mail to tna9@tnadistrict9.com indicating agreement to the nomination. 

Nominations must be received in the TNA D9 office by September 15, 2024.

NOMINATION INSTRUCTIONS

There are 2 ways to submit a nomination: either by using the downloadable form below (the information form would need to be filled out and submitted along with a narrative no more than 2 pages long) or online using the form below (no page or word limit)

§ EXHIBITS EXPERTISE IN PROFESSIONAL PRACTICE

Include:

  • Certification or special award in a specialty area
  • Initiations of policy changes
  • Participation in community projects
  • Advanced education in a specific specialty area
  • Sharing of knowledge through in-services, employee training, etc.
  • Planning and implementation of research or process improvement
  • Development of protocols, guidelines, or projects
  • Other

§ SERVES AS A PROFESSIONAL ROLE MODEL

Include:

  • Active participation in professional associations, on committees, etc. Mentoring other nurses in professional practice
  • Presentation at local, state, and national conferences or business meetings Consultation with other professions and agencies
  • Serving as a spokesperson for professional nursing Serving as an adjunct faculty member or preceptor Policy development
  • Publishing in professional journals or trade publications
  • Other

§ CONTRIBUTES TO THE QUALITY OF HEALTH CARE

Include:

  • Advocacy for patients, students, or employees
  • Improvement of health care, business, or educational delivery systems
  • Changes in policy based on research
  • Political action to support quality health care, management, or education
  • Enhancement of patient access to care or student access to education
  • Development and maintenance of high standards in practice, education, or management Chair or member of institutional committees
  • Outcome evaluation
  • Other

§ COMMITMENT TO THE PROFESSION OF NURSING

Include:

  • Consultation for groups regarding issues and health care delivery, management, or education… Mentoring nurses for leadership in nursing organizations
  • Representation of nursing or management at local, state, and national levels
  • Representation of nursing on community boards (e.g. school boards, community organization boards)
  • Other
