INSTRUCTIONS:

Application must be submitted electronically using the form below and include the following information:

Cover Page

A number will be assigned to all proposals. The cover sheet with the name or other identifying information will be removed so that all reviews will be blind.

Project Proposal

In a total of 5-10 pages, briefly address the following areas:

• Abstract
• Phenomenon or Problem of Interest
• Conceptual or Theoretical Context/Framework
• Project Questions and/or Hypotheses
• Literature Review: Rationale or Background and Significance
• Project Methodology including
  • Design
  • Population Focus/Setting
  • Sample/Sampling Process
  • Data Collection/Instruments
  • Data Analysis Techniques
• Project Budget: Itemization & Justification*
• Project Timeline Completion
• Plan for Dissemination/Publication of Project Results
• References

Project Appendix

• Resume/CV of Investigator(s) including Publications/Presentations of nursing research & clinically relevant projects that address significant but less-explored problems.
• IRB Approval Letter or Form, if appropriate.
• If IRB approval is not required, please provide an approval letter from the agency where the project will be conducted.

*Note: No Indirect Funds will be provided

• Signed Agreement
• Received by Required Date
• Requested Appendices Uploaded
• IRB or Human Subjects Protection Committee Approval Letter, if appropriate, or Agency Approval Letter
